General requirements. The use of a crane man basket is prohibited, except when the erection, use, and dismantling of conventional means of reaching the worksite, such as a personnel hoist, ladder, stairway, aerial lift, elevating work platform or scaffold, would be more hazardous or is not possible because of structural design or worksite conditions, and be written approved by Bayer Construction Manager. 载人吊蓝禁止使用，除非：到达工作位置的通常方法，如梯子、楼梯、高空作业车、高空作业平台、脚手架等的安装、使用、拆除将会带来更大的危险时，或者由于构筑物的设计形状不允许时、施工现场条件不允许时，经过Bayer施工经理书面同意后方可使用。 Project 项目: Location ( attach drawing) 位置（附图） Description of work to be performed 描述要执行的作业： Rated Capacity吊篮额定载重量： Kg. ●Contractor 承包商: Date From日期从: / / To到: / / Time Form时间从: To到: Written safety plan approved？: 书面的安全方案被批准？ 【】 Yes 【】 No Size吊篮尺寸： Crane Leveled in 1% tolerance and Ground Compacte; ●Lifting sensor monitoring computer in cab must work properly; ●Total weight of loaded man-basket and rigging less than 50% SWL of the crane during working. ●Hoisting in a slow, controlled, cautious manner with no sudden movements of the crane. ●Anti-two block device on hook must work properly. ●Brakes (hoist drum, swing, locking) engaged when manbasket in a stationary position.□起重机水平在1%偏差内, 支腿下地面承载力足够。 □起重操作室内全自动力矩限制器工作良好。□吊蓝工作满负荷任何时间不超过吊机允许荷载50％。 □吊钩动作缓慢，始终受控制。□吊车限位器功能正常□吊篮位置稳定时，锁定所有制动装置。 □降落大臂/吊篮时，禁止使用制动器，必须使用档位。 Crane operator吊车司机： Wire rope, shackles, rings, master links, and other rigging hardware must be capable of supporting; ●All eyes in wire rope slings shall be fabricated with thimbles; ● (Use master link or shackle) to ensure the manbasket load is evenly divided; ●Manbasket lifting ropes must be locked in hook; ● Two Tag lines shall be used unless their use creates an unsafe condition. □检查吊环，钢丝绳，卸扣及其他索具的安全额定荷载。□所有钢丝绳端部眼孔必须使用护绳嵌环。 □ (使用组合环或卸扣等) 确保吊篮负荷被平均分配。□吊篮吊索必须被锁在吊钩内。 □必须使用两根溜绳，除非使用将会增加危险。 Rigger/Signalman 司索信号工： ●Manbasket be designed and completed by qualified engineer or authority. Access gates be equipped with a restraining device to prevent accidental opening. In addition to the use of hard hats, employees shall be protected by overhead protection on the personnel platform when employees are exposed to falling objects. The personnel platform shall be conspicuously posted with a plate which indicates the weight of the platform, and its rated load capacity or maximum intended load. □ 吊篮被有资格的工程师或机构设计和制作完成。□ 门设计有防止意外打开的装置。□ 有坠落物体打击可能时，吊篮必须有顶棚。□ 吊篮必须明显位置张贴铭牌，显示有效负荷等信息。 Manbasket inspector 吊篮检查人： ●Overload prohibited●Worker must remain in continuous sight of and in direct communication with the operator or signal person. If not, direct communication walky-talky must be used. ●Safety harness attached to the proper anchorage；Before every time immediately placing personnel to manbasket, A trial lift with fully loaded must be made from location where employees will enter the platform to each location at which the personnel platform is to be hoisted and positioned. ●After trial lifting, the site supervisor will check all safety concerns from related person and responsible for it. ●A meeting shall be held to review the requirements and the procedures. ●weather condition is ok for work: no wind, fog, rain etc.。 □禁止超载。□工人必须始终在吊车司机和信号工视线内，并保持有效沟通, 必要时使用对讲机。□安全带必须正确悬挂。□每次即将进入吊篮之前，从人员进入吊篮起始位置到吊篮停留位置，满负荷的吊篮试验必须进行□试吊后，现场吊篮主管进行安全确认，并对此负责□开工前必须开会明确安全要求与程序。□工作时，天气情况良好。 Site Supervisor Name 现场吊篮主管： Contrctor Project Manager 承包商项目经理 Contractor Safety Manager 承包商安全主管 Bayer Bayre Construction Safety Leader Manager 指定的安全工程师 施工经理 Note : this permit is not available for “Temporarily installed suspended access equipment ” (that is defined in GB 19155-2003, affixed on top of construction building or construction structure, hoisted by wire rope of winch equipment, Please refer to other related safety requirement) 注释：本许可证不适用于：高空作业吊篮 (附着于建筑物或构筑物上，提升机通过钢丝绳驱动，请遵守GB19155—2003和特殊施工设备安全规定) 。 HSE Dept
